The mission of the Green Tree Community Health Foundation is to improve the health status of community residents by initiating and supporting activities in response to identified needs in partnership with community resources.
Established in 2005, Green Tree Community Health Foundation, a not-for-profit public charity, is a community health advocate, whose purpose is to improve the health of Northwest Philadelphia and Eastern Montgomery County. The Foundation does so by identifying areas of vulnerability and partnering with organizations whose work addresses these needs. Through ongoing needs assessments, the Foundation continues to identify needs in these communities and provides funding to organizations whose work will address these needs. Additionally, the Foundation looks for opportunities to develop initiatives where it can fund and provide needed services directly. The Foundation seeks to have a positive impact on current and emerging health issues and risks, empowering residents in its service area to access health services and to value, embrace and maintain their health.
The Foundation's focus is on these specific areas: The frail and isolated elderly; the hungry and food insecure; and children. Where appropriate, the foundation favors programs aimed at prevention.
We fund programs which fit the above criteria and which also serve communities in the following zip codes:
19150 - Wadsworth
19144 - Germantown
19138 - East Germantown
19128 - Roxborough
19119 - Mt. Airy
19118 - Chestnut Hill
The Green Tree Community Health Foundation was created as a result of the sale of Chestnut Hill HealthCare to a joint venture between Community Health Systems, Inc. a for-profit health care system headquartered in Brentwood, Tennessee and the University of Pennsylvania Health System.
Chestnut Hill HealthCare was an integrated health care system including the 100-year-old Chestnut Hill Hospital as well as Chestnut Hill Rehabilitation Hospital, and other related entities.
The Green Tree Community Health Foundation was created to carry out the charitable outreach activities of the former not-for-profit Chestnut Hill Healthcare in its primary service area.
